Instructions for sending a case to Advanced Dental Studio

1. Download perscription form

2. Complete prescription form by filling out the following information:

1. Your name, address, and phone number
2. Patients name and date the case is wanted by
3. Written specifications and directions
4. Signature and lisence number of dentist
5. Product requested
6. Shade requested
7. Design requested
   

3. Pack all necessary materials including: models, bites, impressions, and perscription form

4. Inform Advanced Dental Studio when the case is ready for pick-up at 312.255.9070
